7/30/06  1400hrs
Cottage EMS was dispatched to assist Forest City FD for the missing child in Forest City.  The rescue was also requested to assist with rehab operations at the command center.  The child was found at home a short time later and all units returned to service.

R51 and Cottage EMS units were on hand to standby for the "Ride for Diane" charity motorcycle ride.

7/28/06  2320hrs
Structure fire, Jefferson St. Simpson.  R51 was dispatched after a delay due to a problem with the box alarm to the reported working garage fire along with station 61 and Cottage EMS.  R51 arrived and it's personnel assisted 61 crews with some overhaul.  Cottage EMS provided the EMS standby.

MVA, LVIH between exit's 6 & 7.  R51, Cottage ALS/BLS and CTFD units  were sent to the rollover with injuiries.  R51 and 51-9/CH4 were the  first arriving units to find a vehicle on it's roof with the occupant  already out of the vehicle.  R51 crews stabilized the vehicle and  provided lighting while EMS crews treated and transported the patient  to CMC trauma. Reported pole fire, Railroad St. Whites Crossing.  R51 and Cottage EMS  were sent to assist CTFD units, but were cancelled by C62. R51, 51-9, E60, S62

During the below standby R51 remained in service to respond to  emergencies, and of course, we had one.  R51, Cottage EMS and station  61 were sent on the injury accident at the 700 block of Main in 
Simpson.  Units arrived to, for the second time this year, find a  fight in progress at the scene.  After the fight was broken up R51  provided lighting and assisted EMS crews who transported the injured.

7/21/06  2200-0330hrs
Rescue crews assisted the Carbondale PD and various other police  departments with a DUI check point.  This check point was held in  front of our station.  Cottage provided lighting and other various 
equipment, including an EMS unit.  Crews remained with police units  until almost 4am. R51, CPD

MVA, LVIH between exit's 6 & 7.  Crews were sent for the reported  vehicle rollover with injuries.  R51 and 51-9 were the first to arrive  to find the patient self extricated.  The pt was transported by 51-9

LZ setup, Hendricks.  R51, Cottage EMS & Squad 51 were sent to standby  for the incoming medical helicopter.  R51 set up the daytime LZ and  51-9 transported the crew from Life Flight 4. R51, 51-9, S51

MVA, Brooklyn St. R51 and CFD were dispatched to the reported MVA with  no injuries and fluids down.  R51 remained in service as E51 handled  the hazards. R51, E51, CPD

7/14/06  1645hrs
MVA, Brooklyn St.  The rescue and EMS units were again dispatched to assist the PD along with CFD.  The PD requested BLS only to the scene.  R51 held at station honoring the PD request while E51 continued to the scene.  51-8 transported one to MCH and obtained one refusal.

7/13/06  2310hrs
MVA, Car vs. ATV head on.  The Rescue and EMS were sent along with CPD and CFD for the reported vehicle vs. ATV with the driver of the ATV  unresponsive on the hood of the vehicle.  Units arrived to find the  now conscious ATV operator walking around the scene with a head  injury.  He was treated and transported to CMC by EMS crews while R51  provided scene lighting and helped with cleanup.  The driver of the  vehicle was transported by 51-8 to MCH for an evaluation. MVA, Main & 7th Ave. Carbondale.  R51 and Cottage EMS were dispatched  to an MVA with minor injuries, police requesting BLS only.  Cottage  BLS responded along with E51 while R51 honored the PD request and held  at station. R51, 51-9, E51, CPD

7/6/06  1805hrs
With FCFD's Rescue 41 out of service, R51 was dispatched to assist E42 and FC EMS on the MVA with injuries on Main St. in Vandling.  42 unit's arrived and determined that the rescue was not needed and put back into service. R51, E42, FC EMS

7/5/06  0130hrs
The Rescue was called by the security staff at Marian Community to assist with the removal of handcuffs on a subject.  The cuffs had malfunctioned and needed to be cut off by bolt cutters.

7/1/06 1430hrs
Rescue and EMS sent to assist Station 61 (Fell Twp) to Midland St for  a man stuck on the roof.  Inital reports to units stated that the man  was not injuried, just need help getting down.  Rescue personel 
51-33(Bein) and 51-27(Hollis) headed to the scene.  Engine 61-4  cancelled all units except for EMS.  Male was talked down the ladder  to safety by 61 and 51 members.  Units on the assignment were Engine  61-4, Rescue 51, Squad 62, and Ambulance 51-9.

7/1/06 1730hrs
Rescue and EMS again summoned to assist Station 61 for a hole opening 
up on Geary street.  Rescue arrived on scene to find a mine subsidence 
approximately 4x4 and about 10 feet deep.  Rescue was returned to 
service and EMS was cancelled.  Units on the call were Engine 61-3, 
Rescue 51, Ladder 51, and Ambulance 51-9.  Engine 42 was on standby.

6/28/06  0030hrs
Landing Zone setup at Hendrick's MFG for Lifeflight 3.  Transporting a  patient from the Marian Community Hospital to Danville.  Cottage EMS  assisted flight crew with transport and patient handling.  Units on  call; Rescue 51, Squad 51, and Ambulance 51-9.

Rescue and EMS sent to assist the Carbondale Fire Department for a  fuel oil spill on Terrace St.  Rescue 51 was cancelled en route to the  call.  Units on the assignment were Engine 51, Squad 51, Rescue 51,  Rescue 51-1, and Ambulance 51-9.

Automatic fire alarm, Atlas St. Whites Crossing.  R51 was sent with  Cottage EMS and CTFD for the automatic alarm at a commercial business.    While R51 assembled a crew monitor's 82 (Allison) and 33 (Bein)  responded directly to the scene to learn that there was an alarm  malfunction.  Chief 62 was also on scene and all units were cancelled. MVA, motorcycle vs. car, Belmont St. Carbondale City.  R51, Cottage EMS and CFD were sent to the reported motorcycle vs. car with injuries.  Units arrived to find the motorcycle flipped up on the car with the driver ejected from the bike and over the car.  He was taken to CMC by 51-9/CH4.  The driver of the car refused treatment.  The R51 crew assisted EMS crews with patient care and CFD with hazard control and scene clean up.  R51, 51-9, CH4, E51, CPD Photo 1 and  2

Fuel spill, Main St. Carbondale.  R51 and Cottage EMS responded along  with E51 for the fuel spill at a gas station on Main St.  Units  arrived to find a small gasoline spill.  E51 and R51 crews contained 
the spill.  Cleanup was arranged by the owner. R51, 51-9, E51, EMA 51, CPD

Car Fire, LVIH.  R51 and Cottage EMS responded along with Carbondale Twp. fire for the reported car fire.  Units arrived to find a disabled  car and no fire.  All units returned to service. R51, 51-9, E60, S62, E59

Fire alarm, Main St. Simpson.  R51 and Cottage EMS were dispatched for  the fire alarm at the Fell Charter School.  While enroute all units  were placed back inservice due to the false alarm. R51, 51-9, E61-4, E61-3, L51

Residential structure fire, Washington St. Carbondale. Cottage EMS was  dispatched to the reported house fire along with Carbondale FD.  CFD  units arrived to find a working fire and requested a second alarm  bringing R51 to the scene.  The report of entrapped victims lead to  the request for additional ambulances and manpower by EMS command.   R51/Cottage Hose Co. #2 personnel assisted CFD units with manpower,  including assisting with a primary search of the residence, RIT duties 
and overhaul.  The EMS crews and remaining R51 crew set up rehab and  provided the EMS standby. R51 also provided lighing and cascade.   Rehab 59 was requested by EMS command to assist due to the large  number of fire personnel on scene.  Units remained on scene for almost  3 hours. Brush fire, Center St. Carbondale Twp.  R51 was sent along with Station 60 fire units for the reported brush fire.  Cottage EMS was first on scene and reported that the fire was out.  All units slowed their response.  Units cleared in less that 45 minutes. R51, 51-9, E60, S62

6/17/06  1917hrs
MVA, Jefferson St. Simpson.  R51 and Cottage EMS were initially dispatched to Jefferson St. in Carbondale City along with E51.  As no accident was found units were re-routed to Jefferson St. in Simpson where units found a vehicle into a utility pole with 2 injured.  R51 crews extricated a female victim while EMS crews packaged the self extricated driver.  Both patients were transported by Cottage BLS/ALS to CMC.  R51 remained on scene with 61 fire for scene clean up and awaited the utility companies. 6/17/06  1415hrs
MVA, Rt. 171, Fell Twp.  An injury accident was reported to Wayne Co. Comm. Center by FC EMS on Rt. 171, drawing a response from R51, Cottage EMS and 61 fire.  Units arrived to find a 2 vehicle crash with no injuries, but fluids and debris down.  R51 assisted 61 fire police with a traffic pattern.  FC EMS handled the refusals.  The R51 and E61-4 crews then cleaned up the scene.

R51, R51-2 and Cottage EMS were dispatched to assist station 61 for the reported unknown fire on Rittenhouse St. in Simpson.  Units were cancelled enroute by Chief 61 as the fire was a controlled burn.

MVA, Salem Ave. & Church St.  R51, Cottage EMS and CFD were dispatched to the injury accident.   Units arrived to find one of four patients requiring treatment and transport, which Cottage EMS handled.   R51, 51-9, 51-8, E51, CPD

R51 was requested by Cottage EMS to assist with the CO type call on 43rd St.  R51 crews along with 61 fire personnel cleared the residence of CO, where levels were high enough to require personnel to use SCBA’s. R51, 51-9, CH2, E61-4

The rescue was dispatched to assist the city’s FD and PD with the reported “special needs” person locked in an apartment.  Cottage EMS was also dispatched as precaution.  CFD units gained access to the apartment prior to R51 responding.  51-8, E51, S51, R51-1

Car fire, Fallbrook St.  R51 was dispatched along with Carbondale FD and Cottage EMS for the reported car fire.  R51 did not respond as the initial reports indicated no entrapment.  CFD handled the call with Cottage EMS standing by. 51-9, E51

MVA, Brooklyn St. R51, Cottage EMS and CFD were dispatched for the MVA with injuries.  The R51 crew extricated a non-entrapped patient from the rear seat of the vehicle.  Cottage EMS transported the patient. R51, 51-9, E51, CPD

MVA, Belmont St.  R51, Cottage EMS and CFD were sent to Belmont St. for the reported motorcycle accident.  Units arrived to find a motorcycle in a yard after driving through a chain link fence.  The driver sustained minor injuries and was transported by Cottage EMS. R51, 51-9, CH2, E51, CPD

Tree down, Park St. Carbondale Twp.  The rescue was dispatched along with area 60 fire and Cottage EMS for the tree down in the roadway.  The rescue was cancelled as the fire dept. handled the call.

MVA, 6th Ave. & Main St. R51 and Cottage EMS were dispatched to the MVA with injuries.  Just after dispatch the rescue was cancelled by CFD as the injuries were minor and they requested a BLS unit to check out a patient.  The rescue returned to service.  R51, 51-9, CFD, CPD

6/3/06  0201hrs
MVA, Rt. 6 Carbondale Twp. above Exit 7.  R51 and Cottage EMS were dispatched with area 60 for the MVA with injuries.  Units arrived to find a vehicle over an embankment.  The vehicle had lost control in the construction area, knocked over some traffic barrels and struck a PENNDOT arrow sign before going over a small embankment.  The driver was self extricated and not injured.  EMS crews obtained refusals while the R51 crew provided lighting.  At the request of the PSP troopers R51 crews used the winch to remove the arrow sign from the roadway. MVA, LVIH exit 6 ramp.  R51, Cottage EMS and station 60 fire were dispatched to the MVA, vehicle vs. guiderails.  Units arrived to find all victims extricated and refusing treatment.  The vehicle was wedged under the guiderail. MVA, Gordon Ave.  Cottage EMS was dispatched along with Carbondale FD for the MVA with injuries.  R59 was dispatched to assist R51 as R51 was returning from being serviced.  Units arrived to find a 2 car head on MVA with 2 injuries.  R59 stabilized the vehicles and assisted EMS and CFD crews remove the non-entrapped victims.  R51 arrived a short time later and assisted.  R51, R59, 51-9, 51-8, CH2, CH4, E51, CPD Photo 1and 2

Investigation, Main St. Childs,  R51 was requested by area 60 fire to assist on the investigation on Main St. in Childs for the unknown fire that was originally the call below.  Units determined that the call was a controlled burn and returned to service. R51, S62, 51-8

Structure Fire, R51 and Cottage EMS were dispatched for the reported working fire on Main St. in Simpson.  Shortly after responding units were advised by Chief 61 to return to service as the call was unfounded.   R51, 51-8, L51, S62, E61-4

Car Fire, Jefferson St. Simpson, R51-2 and Cottage EMS were dispatched for the reported car fire.  The R51-2 crew assisted E61-4 and L51 with fire suppression. R51-2, 51-9, E61-4, L51, S62, R51-1

 5/31/06  1800hrs
R51 and Cottage EMS were dispatched to a working house fire along with Carbondale FD on E. Garfield St. in Carbondale City.  CFD arrived to confirm the working fire.  R51 and 51-9 arrived shortly thereafter.  The rescue and EMS crews were immediately summoned by bystanders to the homeowners who were emotionally upset outside the home.  EMS command then  requested a second BLS unit.  After speaking with CFD Chief Brennan, Rehab 59 was requested to assist Cottage with firefighter rehab due to the high temperatures.  R51 set up primary rehab and the joined forces with Rehab 59 upon their arrival.  The first arriving BLS crew stayed with the homeowners to ensure they were ok while the 51-8, 51-7 and R51 crews performed the EMS standby and rehab.  One firefighter was transported ALS to Marian Community by 51-8/CH2.   51, 51-9, 51-8, 51-7, CH2, E51, S51, R51-1, REHAB 59, E24, E59, CPD  More

MVA, Cottage St. at Mermelstein Plaza.  R51, Cottage EMS and Carbondale FD were dispatched to a 3 car MVA with injuries.  Units arrived to find a minor MVA with no injuries.  Cottage EMS obtained refusals.

Main & Salem Ave.  R51, Cottage EMS and Carbondale FD were dispatched for the reported vehicle vs. building.  Units arrived to find a car that backed up over the sidewalk and struck the bank building.  The single occupant was extricated and transported by 51-9.  R51 stabilized the vehicle and assisted the EMS crews. R51 was dispatched along with Station 60 fire and Ladder 58 for the reported "dryer on fire on the side of the road."  The rescue arrived but was not needed as 60 fire knocked it down.   R51, 51-9, E60, S62, L58

Gas Spill, Rt. 6 at the US Mini Mart.  R51, Cottage EMS and Carbondale FD were dispatched to the reported vehicle vs. gas pump with "gas pouring" from the pump.  Units arrived to find approx 25 gallons of gasoline on the ground.  R51 and the units from CFD applied large amounts of absorbant to contain the spill.  The business owner then took responsibility for clean up.  R51 remained on scene for approximately 2 hours.  R51, 51-9, E51, S51, R51-1, CPD, EMA 51, EMA 35-6..... Uncontrolled burn, 44th St. Fell Twp.  R51, R51-2, Cottage EMS and Station 61 were dispatched for the reported uncontolled burn.  Prior to responding all units were cancelled by E61-3 as they were handling the call.   R51, R51-2, 51-9, E61-3, (L51on standby)

MVA, Rt. 6 & Meredith St.  R51, Cottage EMS and Station 60 were dispatched for an MVA with injuries.  R51 arrived and setup a traffic pattern while EMS crews cared for the patients.  R51 then dropped some absorbant and cleaned up the scene. R51, 51-9, 51-8, E60, SQ62

5/20/06  0500hrs
MVA, Belmont St.  R51, Cottage EMS and Carbondale FD were sent for the reported car into a residence.  Units arrived to find a SUV that went off the road, through a retaining wall and a yard, struck a parked car in a driveway, then rested against the porch of the residence.  The victim had minor injuries and was transported by Cottage EMS.  No structural damage was found to the residence.

MVA, Owego Tpk.  R51, Cottage ALS / BLS and Fell Twp. FD were sent to the reported truck rollover.  61 units arrived to find a full size pickup into a tree with the victim self extricated.  The vehicle was stabilized by the E61 and R51 crew.  Both units then provided lighting for the PSP investigated.  The victim refused EMS care.  R51, 51-9, 51-8, CH2, CH3, E61-4, PSP, (R41 & FC EMS on standby)  Photo 1 and 2

ATV accident, South Main St.  R51, Cottage EMS and Carbondale FD were sent for an ATV accident.  Units arrived to find a juvenile that crashed his 70cc motorcycle on the sidewalk.  R51 assisted the EMS crews in packaging the patient. R51, 51-9, CH3, E51, CPD

MVA, Belmont St. R51, Cottage EMS and Carbondale FD were sent to Belmont St. for the reported truck into a building.  Units arrived to find no damage to the vehicle that had parked against a building.  No rescue or fire services were needed. R51, 51-9, E51, CPD

MVA, Canaan & Thorn,  R51, Cottage EMS, Carbondale FD and PD were sent to the reported vehicle vs. utility pole.  Unit arrived to find the uninjured victim who was taken into police custody.  Damage to the utility pole was reported to PP&L. R51, 51-9, E51, CPD

Gas leak, Orchard St. Carbondale.  R51 and Cottage EMS were dispatched to a residence along with Carbondale FD for the reported gas leak. The R51 crew used the multigas meter to check the house while the EMS crew assessed the victim. R51, 51-9, S51

MVA, Belmont St.  R51, Cottage EMS and Carbondale FD were dispatched to an MVA,  vehicle vs. utility pole.  While enroute, EMS was requested to expedite by the Carbondale PD.  Units arrived to find and SUV into a pole with the occupants self extricated.  EMS units transported 2 to CMC with significant injuries. Scott St. for a reported car into a pole.  Units arrived to find that a SUV snapped a pole in half, drove through a fence, crossed the road, over some shrubs into a yard.  The female patient was treated and removed from the scene by 51-9.  R51 returned to service while E51 awaited PP&L. RESCUE / EMS RUN ELEVATOR COLLAPSE
On Tuesday 5/2/06 Rescue 51, Cottage EMS and Carbondale Fire were dispatched to a reported elevator collapse at the Carbondale Housing Authority's north High Rise on N. Main St.  Early reports were that the elevator fell from the 5th floor with one occupant.  Upon arrival of CFD and 51-9 the doors of the elevator were open revealing one injured person inside.  The patient was packaged and removed by Cottage Paramedics, R51 personnel and members of CFD. R51 responded to assist Cottage EMS for a report of a pallet falling onto a man "crushing his head."  While enroute all units were advised that the person had a HAND injury from a tractor trailer truck falling off a jack onto him.  Prior to the arrival of units the patient was removed and transported to the hospital

Structure fire, Fell Twp. R51, R51-2 and Cottage EMS were dispatched along with stations 61, 24 and 51 fire to a working residential fire on Rt. 106. Upon the arrival of R51 heavy fire was showing. R51-2 laid 1000 feet of 5 inch along  with E61-4 and SQ62 totaling almost 3000 feet.
