4/21/08 2100hrs - LZ Setup, Hendrick Mfg.
The Rescue and EMS were sent to set up the LZ for LF3.  Squad 51 provided fire protection while 51-9 transported the crew and patient from MCH.

4/21/08 1544hrs - MVA, Rt. 6 & Canaan St.  Carbondale Twp.
Cottage EMS & Rescue were sent to the MVA with possible injuries and fluids down.  Crews arrived to find a minor MVA with all patients refusing.

4/19/08 1900hrs - EMS Assist, behind Hendrick Mfg.
The Rescue was called to assist 51-9/CH1 in removed an ill patient from the wooded area along the railroad tracks.  The Rescue was canceled, however, as the EMS crew were able to access the patient with an ambulance via dirt path.

4/19/08 1430hrs - Brush fire, Crystal Lake Blvd. Fell Twp
The Rescue and EMS were just clearing the below brush fire when they were dispatched along with Station 61 for another brush fire.  This fire was much smaller and required less resources and time.  Crews cleared in about 30 minutes.

4/19/08 1150hrs - Brush Fire, Porter Ave. Fell Twp.
Just as the crews cleared from the race they were dispatched to assist Station 61 for the "small brush fire" on Porter St. in Fell Twp.  Crews arrived to find a brush fire that eventually spread over 5 acres.  The Rescue was among the first to arrive and quickly extinguished a fire that was spreading through a yard toward a residence.   The EMS crews arrived and set up fire fighter rehab.  R51-2 was dispatched, but staged on the main road, standing by to assist with water supply.    The Rescue crew assisted the FD with fire suppression while the EMS crews provided rehab for over 40 firefighters.  Crews remained on scene for almost 3 hours.  Wm. Walker assisted by covering a call in Carbondale while Cottage crews operated on the fire, as well as multiple other EMS calls.

4/19/08 0830hrs - Standby for YMCA ground breaking and Fred Ciotti Race, Carbondale
The morning started out with Cottage covering the annual Fred Ciotti Memorial Race conducted by the Carbondale YMCA.  This years race was held after the ground breaking for the new $8.1 million dollar YMCA addition.  Several hundred people were on hand for the event.  Four EMS units and the Rescue covered the race with crews becoming busy at the finish line treating patients for heat related illness and dehydration. 

4/18/08 0100hrs - MVA, Casey Highway @ #7
The Rescue was sent along with Cottage EMS & CTFD units for the reported MVA with injuries.  Cottage EMS arrived to find a patient with no injuries refusing treatment.  The Rescue was placed back into service upon it's arrival on the scene.

4/16/08 1515hrs - MVA, N. Church St. Carbondale City
The Rescue, Cottage ALS/BLS, CFD & CPD were sent to the the reported MVA with injuries.  Crews arrived to find that a car had struck a parked car, as well as the occupant of the parked car.  Cottage EMS packaged the patient with the assistance of the Rescue and Squad companies.  He was taken to MCH.

4/15/08 1945hrs - MVA/ATV accident,  Shannon St. Fell Twp.
A vehicle rolled over with 4 patients entrapped sent the Cottage crews up this narrow & steep street.  51-82 (Allison) arrived, established command and reported a Rhino ATV rolled over with no entrapment and four minor injuries.  The EMS crews arrived and triaged, packaged and transported the injured.  Rescue services were not needed.

4/12/08 1800hrs - MVA, Fallbrook St. & Hospital St. Carbondale City
The Rescue and EMS units responded with CFD & CPD for a reported motorcycle accident.  The EMS crews transported the injured to a local hospital.  No rescue services were needed.

4/12/08 1010hrs - LZ Setup, Hendrick Mfg.
The Rescue and EMS were sent to set up the LZ for LF3.  Squad 51 provided fire protection while 51-9 transported the crew and patient from MCH.

4/4/08 0347hrs - MVA, Rt. 6, Childs
The Rescue and EMS were sent to the reported car into a pole with unknown injuries.  R51 and 51-9 were the first to arrive to find a car that struck a pole, knocking it across the roadway with wires down.  The Rescue blocked the roadway and light the scene with the light tower. while the 51-9 crew assessed the self extricated patient, who refused treatment.   Stations 60 and 62 arrived and took over traffic control.  The Rescue and 51-9 were released by command and went back into service.

The Rescue, Squad and 51-9 were sent to set up the LZ for the incoming helicopter.  The call went without incident.

4/3/08 1345hrs - MVA, Belmont St. & Canaan St.  Carbondale City
A two car accident with possible injuries and fluids down brought the Rescue and 2 EMS units through town today.  Crews arrived to find a two car crash with two patient, both refusing.  The Rescue crew stabilized the vehicles and helped E51 with hazard control and cleanup.  The EMS crews obtained the refusals and tended to an elderly patient until family could pick her up.

4/2/08 0420hrs - 2 alarm structure fire, Lincoln Ave. Carbondale City
At around 4:15am on April 2, 2008 Cottage EMS and the Carbondale Fire Dept. were sent to the reported working structure fire on Lincoln Ave in the City.  Crews arrived to find a fully involved fire with heavy fire showing on all sides and divisions.  A 2nd alarm was put out bringing R51 and 51-8 to the scene.   Crews worked for almost 4 hours on the scene with the Rescue providing manpower, lighting and rehab.  Cottage EMS was assisted by Wm. 4/1/08 1545hrs - EMS Assist, Walker Ave. Carbondale Twp.
The Rescue was dispatched to assist 51-9/CH1 for the reported unresponsive person locked in a residence.  The Rescue arrived to find E60 also on scene.  The E60 crew made entry to the home.  The Rescue and EMS crew treated, packaged and removed the patient to the hospital.

3/31/08 0204hrs - LZ Setup - Hendrick Mfg.
The Rescue was called out with S51 and 51-9 to set up the LZ for LifeFlight 3 incoming with an 8 minute ETA.  LF3 was landing just as the Rescue arrived.  51-9 provided transportation.

3/30/08 1455hrs - Brush fire, Salem Mt. Road. Carbondale Twp.
The Rescue was called by Command 62 to the large brush fire involving over two acres that multiple fire depts. were working on.  The R51 crew worked with Cottage EMS and Rehab 59 to rehab all the crews.

3/28/08 1820hrs - LZ Setup, Hendrick Mfg.
The Rescue, Squad and 51-9 were sent to set up the LZ for the incoming helicopter.  The call went without incident.

3/27/08 1355hrs - MVA, N. Church St. & Garfield Ave. Carbondale City
Cottage EMS & Rescue were sent to the reported MVA with injuries and a vehicle smoking.  Units arrived to find that the involved vehicles had fled the scene.  All units returned to service.

3/23/08 13:28hrs - MVA, 8th Ave. & Washington St. Carbondale City
An MVA with injuries brought out the crews on this Easter Sunday.  Crews arrived to find a two car accident with only one injury.  The Rescue crew assisted with the removal of the patient who was transported to MCH by Cottage EMS.

3/21/08 0900hrs - MVA, Dundaff St. Fell Twp.
Cottage EMS & Rescue were sent with Station 61 for the MVA with injuries.  51-9 was the first to arrive to find a car into a tree with the lone occupant self extricated.  They transported the patient to MCH.  The Rescue arrived and controlled hazards while the 61 crew handled traffic.

3/20/08 1700hrs - LZ Setup, Hendrick Mfg.
The Rescue set up the LZ for the Medivac helicopter who came in for the transport out of MCH.  51-8 handled the transportation while S51 provided fire protection.

3/20/08 0900hrs - EMS Assist, Gordon Ave, Carbondale Twp.
A Rescue 51 volunteer was alerted to a possible unresponsive person in a vehicle which prompted a response from Cottage EMS & Rescue.  Crews arrived to find that the call was medical in nature and the Rescue was not needed.

3/15/08 2000hrs - 2330hrs Multiple MVA's Carbondale City
Crash #1 came in just after 8pm and was on Cottage St. just behind our station.  The crews arrived to find two vehicles and six patients.  Four of the patients refused care with 51-9 transporting two patients to MCH with minor injuries.  The Rescue provided lighting with the light tower. 

Crash #2 was dispatched just as the Rescue was backing in the door.  This crash, at Main St. & 7th Ave. in the City involved a van that had gone off Main St. and struck a utility pole.  The handicapped driver was found lying in the back of the van.  The 51 Rescue and Engine arrived,  stabilized the vehicle, launched the light tower and accessed the patient.  The patient was immobilized and removed by crews from Cottage & CFD.  He was transported to MCH by 51-9/CH1. 

Crash #3 came in an hour later as the crews just settled in at home.  A two car crash at Belmont St. & Canaan St. came in just before 11pm.   Crews arrived at this one to find five total patients, two refusals and three minor injuries.  Crews from R51, E51 and Cottage EMS packaged all the patients with 51-9/CH1 & 51-8 transporting the injured to MCH. 

3/11/08 1747hrs - LZ Setup - Hendrick Mfg.
Rescue 51 and Cottage EMS were sent with Squad 51 to set up the LZ for the incoming medical helicopter.  The Rescue set up the LZ, the squad handled fire protection and 51-9 did the transporting.

3/10/08 1430hrs - Wooded area off Exit 6 on the Casey Highway, Area 60
Cottage EMS & Rescue were sent along with the CTFD for a reported ATV accident in the woods.  Units found the accident, but the patient had left.  All units returned to service.

3/8/08 2000hrs - Structure Fire, Justina Dr. Carbondale Twp.
Rescue 51 and Cottage EMS were sent at around 2000hrs on Saturday, March 8th for the reported structure fire on Justina Dr. in Carbondale Twp. 51-9 was the first to arrive to report a two story residence with nothing showing.  E60 and R51 arrived shortly thereafter to learn from the homeowner that the chimney was blocked with a possible fire.  The crews from E60 and R51 headed to the basement while the L58 crew worked outside.  R51 provided lighting.  Crews cleared in just less than an hour after only a blocked chimney was found. 3/8/08 0150hrs - MVA, Rt 106 Fell Twp. 
The Rescue and 2 Cottage EMS units were sent for the 2 car accident with injuries.  The EMS units arrived to find one vehicle that struck the guardrail with no injuries.  The Rescue arrived, deployed the light tower and helped with cleanup.

3/6/08 1958hrs - MVA, Rt 6 @ the car wash, Carbondale City
While awaiting the EMS/Helicopter crew to return from MCH, the Rescue was sent along with CFD for the MVA with injuries.  Crews arrived to find a car into business sign with the driver still in the vehicle.  The Rescue crew stabilized the car, provided lighting and with the help of the CFD and EMS crew extricated the patient.  He was taken to CMC.  The Rescue cleared just in time for the helicopter take off.

3/6/08 1900hrs - LZ Setup - Hendrick Mfg.
The night time LZ was set up for the LifeFlight 3 landing.  After the chopper landed, 51-18 (S. Herchik) was glad he came out for the call because he got to go for a cruise aboard LF3 as the pilot was attempting to burn off fuel.  After flying the length of the valley and getting a bird's eye view of his house the chopper returned and picked up the crew and patient.

3/5/08 1830hrs - MVA, Rt 107 Carbondale Twp.
Cottage EMS & Rescue were sent along with CTFD for the reported MVA with injuries.  Just prior to responding the units were canceled because the crash was in area 36.

3/4/08 1000hrs - Fire Alarm, Reservoir St. Simpson
The Rescue was sent along with Station 61 for the reported automatic fire alarm at the Birch Hills Residence.  Chief 61 arrived and canceled all units.

3/3/08 1700hrs - MVA, Sand St. Carbondale City
Cottage EMS and Rescue were sent to the possible person struck by a vehicle.  The EMS unit arrived to find that the patient was a fall victim.  The Rescue was canceled.  51-9 transported the female to MCH.

Rescue was summoned by a Cottage EMS crew to force entry to a residence where an elderly female was ill inside and not able to answer the door.

2/26/08 18:40hrs - MVA, Jefferson St. Fell Twp.
While at the below call on Rt. 171 the Rescue and EMS were sent along with 61 for a reported car into a tree.  Lt. 51 (Brenzel) responded to the scene with E61-4 to find that the patient had left the scene POV and returned the Rescue and 51-9.

2/26/08 1805hrs - MVA, Rt 171 Fell Twp.
Rescue 51 was diverted from the below call to respond with Station 61 and Cottage EMS for the vehicle rollover  with injuries.  Car 43 (Browndale) was passing by and stopped to assist.  Crews arrived to find the patient in the 43 buggy.  The Rescue provided lighting and the patient was transported to MCH by 51-9.

2/26/08 1755hrs - Car Fire, Casey Highway Carbondale Twp.
Cottage Rescue and EMS were sent along with CTFD units for the reported car fire.  The first arriving units found a fully involved car.  Rescue was en-route but diverted to the above MVA.

2/25/08 2300hrs - MVA, 42nd St. Carbondale City
The EMS unit and Rescue were sent with E51 for the reported MVA, with no injuries but fluids down.  The EMS unit arrived but were returned by the PD.  The Rescue crew assisted CFD with cleanup.

2/25/08 0700hrs - MVA, Casey Highway @ exit 7 - Carbondale Twp.
Captain 51 (Pettinato) called in the injury accident bringing the two EMS units and the Rescue along with CTFD.  The Captain provided care until help arrived.  The Rescue stabilized the vehicles and assisted EMS crews while CTFD controlled hazards.  Cottage EMS transported one to Wayne Memorial and one to CMC.

2/24/08 0459hrs - MVA, Casey Highway @ exit 6.
The Cottage Rescue and EMS were sent for the car over an embankment, on it's roof in the area of exit 6.  Chief 62 arrived at the area to find that the vehicle had left the scene.  All unit were canceled.

2/24/08 0418hrs - MCI, Interstate 380 - Covington Twp.
The Cottage MCI trailer was requested along with multiple other EMS agencies for the Greyhound bus that had rolled over with 40-50 patients and entrapment.  The trailer and an ALS unit rolled within minutes with a total crew of 6.  Upon arrival the crew was requested right into the scene.  The crew remained for about an hour.  The ALS unit transported two patients to Mercy.

2/24/08 0405hrs - MVA, Brooklyn St. Carbondale City.
The Rescue and EMS units were sent to Brooklyn St. just down from our station for the reported rollover with one patient laying in the roadway.  Upon the arrival of the Rescue it was returned and the crew pro ceded to the above call.  51-9./CH1 transported one patient to MCH.

2/10/08 14:52hrs - MVA, Casey Highway at exit #7
Cottage EMS & Rescue were sent for the injury accident.  Squad 62 was the first to arrive and reported a minor accident with no injuries.  The Rescue canceled and 51-9 went in for the patient refusals.

2/9/08 2240hrs - MVA, Rt 106 Fell Twp.
Rescue 51, Rescue 51-2 and Cottage EMS were sent to the reported accident with one injured.  Units arrived to find a two car crash with all occupants out of the vehicle and refusing treatment.  The recovery service was on scene and all units were returned.

2/9/08 1500hrs - MVA, Roosevelt Highway, Carbondale Twp.
Cottage Rescue and EMS were sent for the report of a car off the road "teetering" on a snow bank.  Station 62 monitors arrived to find no accident, but a car that slid off the road and was already removed.  All units returned.

2/8/08 1500hrs - MVA, Pike St. & Gordon Ave. Carbondale City
An accident with one injury brought out the Rescue and 51-9 today.  Units arrived to find the victim still in her vehicle.  The Rescue stabilized the car and assisted with the removal of the patient.  She was taken to MCH by 51-9.

2/3/08 2300hrs - MVA, Upper Powderly St. Carbondale Twp.
Shortly after the Superbowl ended the car into a pole brought the Cottage crews out along with CTFD units.  E60 was the first to arrive with Captain 51 and R51 arriving shortly after.  They found one victim with a head injury self extricated.  The Rescue crew packaged the patient and provided care unit the EMS unit arrived.  R51 also provided lighting.  CTFD units handled hazard control.  51-8 transported the victim to MCH.

2/1/08 1530hrs - MVA, Rt 171 Fell Twp.
Stations 41, 42 & 43 were originally sent along with Forest City EMS for the reported accident in area 42.  Chief 42 arrived and reported that accident in area 61 bringing the Cottage Rescue.  Mobile 9 was already en-route.  A car that slid off the road into a tree caused minor injuries and minor damage.  To reduce traffic congestion, command returned all units except M9, FC6, E61-4 and FP42.

1/31/08 2030hrs - MVA, Rt 171 Fell Twp.
Cottage EMS & Rescue were dispatched to the reported car into a house with entrapment.  51-61 (A. Scotchlas) was one of the first to arrive and reported a vehicle on it's roof with one patient on the ground, no entrapment.  51-9 arrived and began patient care and packaging.  The Rescue arrived with 6 and provided lighting, assisted the EMS crew and helped cleanup. 51-9/CH1 transported the female to CMC.  Units remained for about an hour.

1/27/08 2130hrs - Car Fire, Rt. 106 Fell Twp.
Rescue and EMS were sent along with Station 61 for the reported fully involved car fire. 51-9 was the first unit to arrive and confirmed the fully involved car.  The rescue arrived and assisted with lighting from the light tower and manpower.  Units cleared in about an hour.

1/22/08 1424hrs - Gas leak, Homestead St. Simpson
Rescue 51 was dispatched along with 51-9 and Station 61 for the reported gas leak due to a construction company striking a line.  Due to possible daytime manpower shortages, R51-2 was also requested to the scene.  R51 was the first to arrive to find that the construction company had controlled the leak and the gas company was on scene.  Units returned in less than 15 minutes.

1/18/08 1221hrs - EMS Assist, Main St. Carbondale City
While all Cottage EMS units were on numerous EMS calls, Rescue 51 and Wm. Walker EMS were sent to the reported fall victim.  The rescue responded from the below helicopter landing.  Cottage volunteers were able to staff another BLS unit and respond so 59-9 was canceled.  The rescue crew provided patient care until the EMS unit arrived.

1/17/08 1450hrs - 1522hrs - MVA, Dundaff St. Fell Twp.
Cottage Rescue 51, Rescue 51-2 and Cottage EMS were sent to the top of Dundaff St. in Fell Twp. with Station 61 for the reported car into a pole, with the pole down.  51-9/CH1 were the first to arrive to report a car through a pole, off the road with one non-entrapped occupant still in the vehicle.  The Rescue arrived with the crew assisting the EMS crew extrication of the victim.  R51-2 arrived and controlled traffic with FP61.  Cottage EMS transported the injured to Marian Community with minor injuries.
